Below are the attached contracts and forms that need to be filled out to become a member. The required forms include:

1. Residential Application
2. Right-of-Way Easement
3. Receipt of Disclosures

The optional forms include:

1. Operation Round Up
2. Third Party Designation
3. Security Light Agreement

The forms that are for your records include:

1. Non-Discrimination Statement
2. OMEC Bylaws
3. OMEC Rules and Regulations

Please note that the Right of Way Easement form requires the signatures of all parties listed on the deed of the property AND a notary stamp and signature and will NOT be accepted if signed without a notary. Oneida-Madison Electric has a notary on staff and is happy to assist you in person at our office during regular business hours (Monday-Friday, 7:30am-4pm)

Additional Membership Requirements

1. a driver's license or government-issued photo ID
2. $10 membership fee
3. in the case of a short-term rental, a consumer deposit may be required. This deposit will be returned upon 12 consecutive, in-full, on-time payments, or in the event service is terminated, whichever comes first.
